We investigated the impacts of acetyl L-carnitine (ALC) on chronic fatigue in elderly patients, examining 96 individuals aged 70 and above. The study utilized fatigue scales to assess changes after treatment.
Results indicated that ALC significantly reduced both physical and mental fatigue while improving cognitive function and overall physical status. Participants reported a notable decrease in muscle pain and sleep disorders, alongside improvements in fatigue severity.
Overall, ALC shows promise as a treatment option for alleviating chronic fatigue in older adults.

Acetyl L-carnitine shows promise
Exploratory open label, randomized study of acetyl- and propionylcarnitine in chronic fatigue syndrome.
We explored the impact of acetyl L-carnitine, propionyl L-carnitine, and their combination on chronic fatigue syndrome (CFS) in a group of 90 patients over 24 weeks.
The results indicated significant improvements in fatigue and attention concentration, with acetyl L-carnitine particularly benefiting mental fatigue and propionyl L-carnitine focused on general fatigue.
However, those receiving both treatments showed less improvement. It’s also worth noting that many patients experienced a return of fatigue after treatment ended.

Acetyl L-carnitine shows promise
A randomised controlled trial comparing duloxetine and acetyl L-carnitine in fibromyalgic patients: preliminary data.
We analyzed a study that compared duloxetine and acetyl L-carnitine in treating female patients with fibromyalgia syndrome (FMS).
Over 12 weeks, the 65 participants took either duloxetine or acetyl L-carnitine, with evaluations conducted to assess their symptoms.
While both treatments improved overall well-being and pain levels, only duloxetine showed significant effects on anxiety and psychological aspects of quality of life.
Thus, acetyl L-carnitine shows promise but may not offer significant benefits for anxiety improvement in FMS patients.

I suspect I have CFS, and this product improved my condition remarkably within a day. I reduced my sleep requirement from 9.5 hours to about 8.5 hours and felt more energetic with fewer headaches. Discovering that I had symptoms of CFS was surprising; I always thought I had depression, but anti-depressants weren't effective. This treatment has been a miracle for me. If you're persistently tired, give it a go—you have nothing to lose but a bit of money.

ALCAR improves MS fatigue tolerance
Comparison of the effects of acetyl L-carnitine and amantadine for the treatment of fatigue in multiple sclerosis: results of a pilot, randomised, double-blind, crossover trial.
We conducted a clinical trial to see how acetyl L-carnitine (ALCAR) stacks up against amantadine for treating fatigue in multiple sclerosis patients. In this study, 36 patients were given either ALCAR or amantadine for three months and then switched to the other treatment after a washout period.
We found that ALCAR showed greater effectiveness in reducing fatigue, as measured by the Fatigue Severity Scale. However, it did not show significant benefits in other areas like depression or social experiences. Overall, ALCAR was better tolerated than amantadine, making it a promising option for managing fatigue.
