We investigated the intriguing connection between vitamin B12 and autoimmune neurologic disorders, focusing on patients with symptoms like tremors and ataxia. Through advanced techniques like programmable phage display, we identified an autoantibody against the transcobalamin receptor, CD320, in patients who exhibited significant neurological deficits.
This autoantibody hampers the cellular uptake of vitamin B12, leading to strikingly low levels of the vitamin in the cerebrospinal fluid, even when serum levels appeared normal. Despite the challenges posed by the presence of this autoantibody, treatment with high-dose vitamin B12 and immunosuppressants showed promising results, as we observed increased levels of B12 in cerebrospinal fluid and notable clinical improvement in patients.
Our findings also indicated that similar autoantibodies appeared in other patients suffering from unexplained neurologic conditions, making vitamin B12 deficiency a key factor worth exploring in such cases. However, despite the elevated levels of vitamin B12 observed in cerebrospinal fluid post-treatment, the patients initially showed no signs of typical hematologic vitamin B12 deficiency.
This study sheds light on the complex nature of vitamin B12 transport in the body, revealing a potential avenue for treating autoimmune neurologic disorders through focused immunomodulatory therapy and vitamin supplementation.

Vitamin B12 and autoimmune gastritis
Risk Factors Analysis and Predictive Model Construction for Autoimmune Gastritis: A Nationwide Multicenter Case-Control Study in China.
We investigated the dietary and lifestyle factors associated with autoimmune gastritis (AIG) among Chinese patients. Our study involved 479 individuals across seven centers in China, where we collected detailed clinical and lifestyle data.
From the findings, we noticed that most AIG patients were middle-aged females. Interestingly, our analysis highlighted that a diet rich in vitamin B12 appeared to play a protective role against AIG. This means that the more vitamin B12 included in the diet, the better the potential outcomes regarding autoimmune gastritis.
Furthermore, we established a predictive model that relied on various factors. Alongside vitamin B12, using larger amounts of cooking oil and having comorbid autoimmune thyroid disease emerged as key risk factors for developing AIG. The predictive model demonstrated promise, with ROC curve scores indicating a reasonable ability to differentiate AIG patients from others.
Overall, our exploration suggests that dietary habits, including vitamin B12 intake, can be pivotal in managing the risk and prognosis of autoimmune gastritis. However, we recognize that additional research is essential to fully understand the effects of vitamin B12 in this context.

Vitamin B12 inhibits peptidylarginine deiminases and ameliorates rheumatoid arthritis in CAIA mice.
We investigated the potential of vitamin B12 as a treatment for rheumatoid arthritis, particularly focusing on its ability to target an enzyme called peptidylarginine deiminase isoform 4 (PADI-4). PADI-4 plays a crucial role in a process called citrullination, which is often disrupted in autoimmune conditions like rheumatoid arthritis.
Our findings revealed that vitamin B12 effectively inhibits PADI-4, showing a preference for this enzyme over another similar one, PADI-2. This inhibition corresponds to a decrease in overall cellular citrullination, including the citrullination of histone H3, which is also mediated by PADI-4.
Additionally, we observed that hydroxocobalamin, a special form of vitamin B12, significantly reduced the severity of collagen type II antibody-induced arthritis in mice. It also lowered the expression of key inflammatory factors and cytokines associated with rheumatoid arthritis, such as IL-17A, TNFα, and IL-6.
These promising results suggest that vitamin B12 might be an effective therapeutic avenue for managing rheumatoid arthritis, warranting further investigation in future studies.
