Iron treatment improves leg cramps
Iron deficiency associates with deterioration in several symptoms independently from hemoglobin level among chronic hemodialysis patients.
Relevant but not conclusive findings.
We explored the effects of iron deficiency (ID) in patients on maintenance hemodialysis, especially regarding common symptoms like leg cramps. The study included 154 patients, all of whom had reached target hemoglobin levels, which means they were not anemic. This allowed us to focus on the impact of ID without the confounding factor of low hemoglobin.

Our findings revealed that patients with iron deficiency reported a higher severity of leg cramps compared to those without ID. Specifically, we observed that a low transferrin saturation (TSAT) level contributed significantly to the experience of pain during vascular access punctures and those annoying intradialytic leg cramps. Additionally, patients with lower ferritin levels faced more intense issues like severe fatigue and headaches during their dialysis sessions.

The results suggest that treating iron deficiency could potentially alleviate these critical symptoms, indicating that proactive iron supplementation may be beneficial for those undergoing maintenance dialysis. Addressing ID might not only improve hemoglobin levels but also enhance overall quality of life by reducing painful experiences like leg cramps.
