We explored the use of ginger-derived extracellular vesicles modified with folic acid to treat rheumatoid arthritis, a painful autoimmune condition. While higher MTX doses increased the risk of needing to stop treatment due to adverse effects, adding folic acid seemed to lower that risk significantly.

In fact, around 51.7% of patients had to discontinue MTX within 15 years, but those on folic acid had a much better outcome.

PANS/PANDAS and folate treatment
We aimed to understand how folate receptor alpha autoantibodies (FRAAs) relate to symptoms in children diagnosed with Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections (PANDAS) and Pediatric Acute-Onset Neuropsychiatric Syndrome (PANS). We analyzed serum samples from 47 children, aged 6 to 18, who carried these diagnoses.

Our results showed that a significant number of these children—about 63.8%—had binding or blocking FRAAs. Additionally, certain symptoms, like severe tics, linked with higher FRAA binding titers. Notably, we observed that kids with autism spectrum disorder (ASD) had lower binding titers of FRAAs.

A remarkable case highlighted how treatment with leucovorin, a form of folate, resulted in symptom improvements in a child with PANS/PANDAS who tested positive for FRAAs. These findings suggest that abnormalities in folate metabolism could play a role in the symptomatology of PANS/PANDAS, presenting the potential for folate-based therapies to be a viable treatment option.

Overall, this study sheds light on the connection between folate-related factors and pediatric neuropsychiatric disorders, calling for further research into leucovorin as a targeted treatment.

Methylfolate is an effective bioactive folate form. Due to limited conversion of synthetic folic acid, 5-MTHF should be taken for proper absorption, especially for pregnant women or those with elevated homocysteine levels linked to an autoimmune disorder. This affordable methylfolate may help in addressing those deficiencies.

We looked into how folate, an essential nutrient in our diet, might influence the health of individuals with systemic lupus erythematosus (SLE), an autoimmune disorder that carries a heightened risk of cardiovascular disease. Our investigation involved a case-control study with 394 Mexican-mestizo women, where we sought to find connections between certain genetic variants related to folate metabolism and heart health in SLE patients.

Surprisingly, our results showed that the overall levels of folate and homocysteine—both important in the context of heart health—didn't differ significantly between those with SLE and healthy controls. However, we did observe that the genetic variant c.+677 C>T (known as the CC genotype) appeared to offer some protective benefit against cardiovascular issues, suggesting that people with this genetic makeup might have a lower risk of heart disease.

On the flip side, two specific haplotypes from the c.+677 and c.+1298 genetic variants were linked to an increased risk for SLE itself. This suggests that while folate's role in the body is vital and its genetic connections intricate, treating SLE patients with folate alone may not yield significant improvements in their condition based on these findings.

Overall, while we documented interesting associations between genetic variants and disease risk in SLE, we didn't find strong evidence supporting direct benefits of folate treatment in mitigating the disorder's impact.
