Causal link: Folic acid and ulcers
Our investigation focused on the potential benefits of folic acid (FA) supplementary therapy in reducing the risk of gastric ulcers (GU). Using a method called Mendelian randomisation, we tapped into data from the UK Biobank, which provided a rich source of information for our analysis.

The analysis involved identifying seven genetic markers associated with FA supplementation. Through robust testing that included several statistical methods, we found that those genetically predisposed to higher FA intake had a significantly lower risk of developing GU.

Specifically, we observed that the odds of having a gastric ulcer dropped by about 13% among participants who were genetically linked to higher levels of FA supplementation. This finding remained consistent across various statistical approaches, reinforcing our confidence in the results.

Furthermore, we confirmed that no single genetic marker skewed the overall results, suggesting a reliable association. Overall, this study provides compelling evidence that supplementing our diet with folic acid could play a protective role against gastric ulcers.

Effects of folate on ulcerative colitis
We delved into how folate treatment, specifically through folate-chicory acid liposomes, affects ulcerative colitis (UC). This study involved both laboratory models that mimic inflammation caused by LPS and a mouse model of UC induced by dextran sulfate sodium (DSS). Our goal was to see whether these specialized liposomes could help target macrophages and improve symptoms associated with UC.

Our findings revealed that the folate-chicory acid liposomes not only targeted macrophages effectively but also exhibited high encapsulation efficiency and a manageable particle size. In laboratory tests, we observed that they reduced inflammation in macrophages and controlled the expression of key inflammatory markers. When applied to UC mice, these liposomes showed promising results by alleviating clinical symptoms, increasing colon length, and reducing histopathological damage.

We noted that the treatment worked by inhibiting the polarization of macrophages towards the harmful M1 phenotype while downregulating the TLR4/NF-κB signaling pathway, which is often associated with inflammation. Overall, the folate-chicory acid liposome demonstrated significant therapeutic potential for managing ulcerative colitis.

Genetic insights into ulcer healing
We delved into the complexities surrounding a specific genetic mutation in the LMBRD1 gene, which is linked to a variant form of cobalamin (vitamin B12) deficiency. This study highlighted a 15-year-old male with symptoms typical of Dyskeratosis Congenita, including skin hyperpigmentation and ulcers. After identifying a novel splice site deletion in the gene, we learned that this alteration can heavily impact vitamin B12 metabolism and lead to various clinical manifestations, including ulcers.

Importantly, we observed the effects of administering intravenous hydroxocobalamin therapy. Following treatment, our findings indicated a notable improvement in the patient's skin hyperpigmentation, angular cheilitis, and the painful aphthous ulcers. While the study primarily focused on genetic factors and treatment responses, the results suggested that vitamin B12 therapy plays a valuable role in alleviating ulcer symptoms. We encourage further exploration to expand our understanding of these relationships and optimize treatment approaches.

Vitamin B12's Role in Oral Health
We explored the connection between recurrent aphthous stomatitis (RAS), commonly known as oral ulcers, and various hematological parameters, particularly focusing on vitamin B12. Our systematic review and meta-analysis brought together multiple studies to identify how deficiencies in this vitamin and other blood-related factors might influence the formation and recurrence of these painful ulcers.

From our analysis, we discovered that individuals suffering from RAS had significantly lower levels of vitamin B12 and hemoglobin compared to healthy individuals. Specifically, our findings showed that participants with vitamin B12 deficiency were nearly three times more likely to develop RAS than those without the deficiency. This suggests that vitamin B12 may play a crucial role in maintaining oral health and preventing the occurrence of these ulcers.

The study highlights that addressing vitamin B12 levels could be a beneficial strategy in managing RAS. By ensuring adequate levels of this essential vitamin, we might reduce the frequency and severity of ulcers that many deal with regularly. Overall, our research emphasizes the importance of monitoring and potentially supplementing vitamin B12 for those experiencing recurrent oral ulcers.

Golden kiwifruit aids gastric healing
We examined the effects of hydro-alcoholic extracts from golden kiwifruit flesh and peel on ulcers caused by indomethacin in rats. The study involved several groups, including a control that received just water and one that received indomethacin to induce gastric ulcers.

Results indicated that golden kiwifruit extracts reduced stomach damage and improved healing compared to controls. Notably, the extracts elevated vitamin B levels and showcased antioxidant properties. While Lansoprazole also helped, golden kiwifruit extracts showed even greater potential benefits for ulcer treatment.

We observed a case involving a 27-year-old male with a challenging condition known as pyoderma gangrenosum, a type of ulcer that can develop after surgeries like gastric bypass. In treating his non-healing leg ulcer, the medical team combined various approaches, including systemic immunomodulators and surgical intervention.

As part of his management, he received vitamin B complex, vitamin D supplements, and zinc sulfate, along with folic acid. While we noted the inclusion of folate in his treatment regimen, the overall effectiveness of folate alone on ulcer healing remains unclear due to the multifaceted treatment approach used.

The healing process was significantly aided by administering multiple doses of Infliximab, which is important to highlight. Therefore, while we recognize the role of folate in supporting overall health, the study does not isolate its specific impact on the ulcer’s healing.
