We aimed to understand how calcium levels in saliva are affected in individuals with hypothyroidism. As one of the most common thyroid disorders, hypothyroidism can lead to various oral health issues, including a higher likelihood of dental caries and periodontal disease.

Our study measured the salivary calcium levels in patients diagnosed with hypothyroidism and found that these levels were significantly lower compared to healthy individuals. It's important to note, however, that we did not find any meaningful connections between the salivary calcium concentrations and the levels of thyroid hormones (TSH and T4) in the blood.

While a decrease in salivary calcium might explain the increased dental problems in hypothyroidism patients, the study did not explore any potential benefits of calcium treatment for this condition. We explored the case of a 48-year-old woman who faced compressive symptoms due to a rapidly growing goiter, tied to her existing hypothyroidism. Despite her hypoparathyroidism being revealed through biological tests, she did not exhibit any signs of hypocalcemia initially.

The treatment she received included levothyroxine, calcium, and alfacalcidol, but a complete thyroidectomy was deemed unnecessary due to the tumor’s rigid adhesion to nearby structures. Upon histopathological examination, her condition was identified as Riedel’s thyroiditis. We watched as the introduction of glucocorticoids resulted in a significant reduction of symptoms and shrinkage of the thyroid gland.

Notably, after discontinuation of the vitamin and calcium supplementation, her serum calcium and parathyroid hormone levels normalized. This suggests that, in some instances, hypoparathyroidism can remain asymptomatic, especially under conditions like Riedel's thyroiditis. Furthermore, early glucocorticoid treatment seems valuable in addressing associated fibrosclerosis and restoring parathyroid function.

While calcium was part of the therapeutic strategy, it is challenging to ascertain its isolated effect due to the combination with other treatments in this specific case.
