We explored the effects of L-carnitine on patients suffering from cystic acne who were undergoing isotretinoin therapy. This study involved 230 patients, all treated with a standard isotretinoin dosage. When they began to experience muscular symptoms, we examined the role of L-carnitine supplementation. Half of the patients received L-carnitine, while the other half received a placebo.

The results were encouraging for those on L-carnitine. Patients who received the supplement reported a notable improvement in muscle pain and discomfort within just a week. Additionally, their liver enzyme levels returned to normal, along with an increase in carnitine levels. In contrast, the placebo group continued to struggle with muscle symptoms, highlighting the potential benefits of L-carnitine for these patients. We investigated the potential of L-carnitine combined with salicylic acid, referred to as IP-BHA, and magnolol to combat acne effectively. Traditional salicylic acid has been useful for acne treatment due to its ability to reduce skin cell buildup and kill bacteria. However, we aimed to enhance its performance and reduce skin irritation, especially at neutral pH levels.

Through our research, we found that the combination of IP-BHA and magnolol significantly improved anti-acne effects. Notably, magnolol demonstrated robust antibacterial activity against Cutibacterium acnes, surpassing that of a widely used antibiotic, clindamycin. When paired with IP-BHA, the antibacterial effectiveness increased by 2.5 times, showcasing the strength of their combined properties.

We also observed the duo's positive impact on reducing lipogenesis triggered by testosterone, while independent treatment showed minimal effects. Additionally, inflammatory markers linked to C. acnes were completely inhibited when using both IP-BHA and magnolol together. These powerful ingredients not only help in exfoliating the skin but also soothe inflammation linked to acne.

Our study highlights that the ionic pair of L-carnitine and salicylic acid can be effective against acne while remaining gentle on the skin. The findings suggest promising opportunities for using IP-BHA and magnolol as a treatment option for improving inflammatory acne conditions.

Combination limits L-carnitine analysis
We evaluated the effectiveness of a moisturizer containing licochalcone A, 1,2-decanediol, L-carnitine, and salicylic acid for preventing acne relapse during the maintenance phase in Thai patients with mild to moderate acne. In a controlled clinical trial, after an initial treatment with a combination of adapalene and benzoyl peroxide, patients who saw significant improvement were split into two groups.

Each group applied either the moisturizer or a vehicle lotion without active ingredients to different sides of their faces for 12 weeks. By looking at acne lesion counts and the overall severity of acne, we observed that the treatment group using the active moisturizer had notably fewer lesions after the trial period.

Importantly, we found no significant differences in skin dryness or irritation between the groups, suggesting the product was well-tolerated. The inclusion of L-carnitine in the moisturizer seems to contribute positively to reducing acne lesions and preventing new ones during this maintenance phase, even if we cannot determine its isolated effects due to the combination method used in the study.

We conducted a study to assess how a daily routine involving Licochalcone A, Salicylic acid, and L-Carnitine can improve mild facial acne. Over eight weeks, 91 adults applied a fluid with Licochalcone A, Salicylic acid, and L-Carnitine in the morning and a cream with Licochalcone A and Hydroxy-Complex 10% at night.

Our effectiveness measurements used the Global Acne Grading System (GAGS) and counted comedones (clogged pores) and papules (small raised bumps). By the four-week mark, we observed a statistically significant decrease in GAGS scores, along with a 41% reduction in comedones and a 45% decrease in papules. Additionally, there was a notable drop in sebum production by about 47%.

By the end of the eight-week treatment period, these positive trends persisted, showcasing further reductions of 64% in GAGS scores and 71% in both comedones and papules. This suggests that the combination regimen we explored—featuring L-Carnitine as a key ingredient—presents a promising option for those dealing with mild acne, offering a pleasant approach to skin care.
