We delved into vitamin B12 deficiency and its potential to cause dizziness, particularly in cases that mimic hemolytic anemia. A 60-year-old woman presented with dizziness, weakness, and other troubling symptoms. After evaluation, we found she had a severe deficiency linked to pernicious anemia.

Vitamin B12 shows mixed effectiveness
We examined the effectiveness of mecobalamin, a form of vitamin B12, combined with vestibular rehabilitation training in treating acute vestibular neuritis—a condition often accompanied by dizziness. In our literature review, we scoured various databases for studies on this combination therapy to see how it could improve symptoms associated with dizziness.

Our analysis included several studies with a low risk of bias. We found that while mecobalamin alongside rehabilitation training seemed to help reduce some symptoms of dizziness, it did not significantly improve balance confidence among the patients when compared to rehabilitation alone.

Notably, we observed a marked decrease in dizziness handicap scores for those receiving the combined treatment after six months, suggesting that mecobalamin may help relieve dizziness in some patients. However, the results were mixed, and more research is needed to understand the isolated effects of vitamin B12.

Overall, our investigation highlights that while vitamin B12 might have a role in alleviating certain dizziness-related symptoms, its effectiveness, especially in combination with rehabilitation, isn't fully conclusive yet. Continued exploration in this area could shed additional light on how we approach treatment for vestibular disorders.
