Our findings revealed that vitamin B12 plays a crucial role in programming microglial cells—immune cells in the brain—helping them divide and activate effectively, which is vital during the healing process. This treatment appeared to improve the metabolism of these cells as well, promoting better functionality and reduced brain injury after a stroke.

Interestingly, we found that vitamin B12 significantly limited gene alterations associated with strokes in these microglia. Throughout the acute phase of an ischemic stroke, vitamin B12 altered the activities of different microglial subsets, leading to less brain damage and enhanced neurological recovery for the mice in our study. However, when we depleted microglia before administering the stroke, the beneficial effects of vitamin B12 disappeared, highlighting the importance of microglia in this process.

Overall, our research underscores the potential of vitamin B12 in mitigating neuroinflammation and aiding recovery for stroke victims. Through our preclinical studies, we've established that vitamin B12 may indeed offer a promising avenue for improving neurological outcomes post-stroke, meriting further exploration.

Role of vitamin B12 and folic acid in treatment of Alzheimer's disease: a meta-analysis of randomized control trials.

We explored the potential role of vitamin B12, alongside folic acid, in slowing the progression of Alzheimer's disease (AD) through a meta-analysis of randomized control trials. Our aim was to determine whether these supplements might improve cognitive function, daily life activities, and influence blood homocysteine levels in individuals diagnosed with AD.

After carefully sifting through studies, we found five that met our criteria. These studies assessed the cognitive changes in participants through the Mini-Mental State Examination (MMSE) and the Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-Cog). Additionally, they examined the impact on daily functioning as well as blood homocysteine levels.

The results showed that after six months of treatment, participants taking vitamin B12 and folic acid did experience a slight improvement in MMSE scores compared to those on a placebo. However, there was no significant change in the ADAS-Cog scores, which means some cognitive aspects didn’t show marked improvement. Likewise, daily life functioning did not see any benefits from the supplements.

Importantly, we also noted a significant reduction in blood homocysteine levels following the supplementation, suggesting a biochemical effect of the treatment. Overall, while vitamin B12 shows promise in enhancing some cognitive measures, it does not translate directly into broader cognitive or daily functioning improvements in Alzheimer's patients.

We explored a group of 29 patients who were referred to three allergy centers in the UK due to suspected vitamin B12 hypersensitivity (VB12H). Over the period from 2014 to 2022, these patients underwent various tests, including skin prick tests and drug provocation tests. The aim was to understand how vitamin B12 treatment relates to allergies and to develop a reliable method for assessing hypersensitivity.

Among the patients, 62% displayed immediate hypersensitivity reactions, with some experiencing severe anaphylaxis to vitamin B12 forms. Interestingly, we found that some patients who were sensitized to a specific type of vitamin B12 could tolerate another form. This indicates that not all vitamin B12 treatments may cause problems for everyone. However, confirmed cases of VB12H are considered to be quite rare.

Overall, while we identified individual cases where vitamin B12 treatment provoked allergic responses, the evidence suggests that vitamin B12 hypersensitivity is uncommon. This nuances the discussions around vitamin B12 treatment face since adverse reactions are not widespread. Our proposed evaluation protocol includes skin testing and considerations for allergies to related substances, such as polyethylene glycol.

Therefore, while there is an observable connection between vitamin B12 and allergic reactions in a small subset of individuals, the study indicates that treatment for vitamin B12 deficiency remains generally safe for most individuals.
